Here is an old-school-styled Character Checklist available free on DriveThruRPG for the 5th edition of the World's Most Popular Role-playing Game. I have made many versions of these styled after the old Judges Guild Character Checklist.
The information on the sheet helps a game judge by keeping track of basic details for each player character. This helps speed up tasks during combat and also helps the game judge make rolls in secret during a party's exploration confidentially without alerting the party. An added bonus is there are at least minimalist details for each member of the adventuring party. In the event a character sheet is forgotten for a game or lost/destroyed that details here could assist in rebuilding a new sheet.
(I also updated here a very simple old-school-style character sheet for the 5e 2024 revision. No one in my game have used it yet, so I haven't tested playability.)
